phy-1Dr. Ami Maniar , a Physiotherapist , passed B.physiotherapy in September ‘99 from Maharaja Sayaji Rao University , Baroda. She is a lifemember of IAP. (Indian Association of Physiotherapists) . She has an experience of 15 years in the field of physiotherapy.She deals with all kinds of musculoskeletal, orthopaedic, neurological, paediatric physiotherapy disorders. Special Interest and work experience in Antenatal and Postnatal physiotherapy ( i.e. pre-pregnancy and post¬-pregnancy classes) and cerebral palsy clinics.

She is specialised in Manual Therapy techniques for the spine and joints. She is specialised in Kinesiology Taping for all conditions. She has completed course in Food and Nutrition (IGNOU) Delhi and Hospital administration from NILEM (Chennai).

 

What is physiotherapy?

With more moving parts than any machine, it is not surprising that your body cries out in pain or refuses to do what you tell it .There are hundreds of different muscles and joints that can go wrong. Fortunately, there is an effective way to treat many problems you may experience-Physiotherapy.

  • Physiotherapy uses a variety of techniques to help your muscles and joints work to their full potential, helping to repair damage by speeding up the healing process and reducing pain and stiffness.
  • Physiotherapists also have an important role in rehabilitation, such as helping people who have had strokes to relearn basic movements.
  • Physiotherapists do not simply offer treatment – their advice can help you prevent problems returning or even happening in the first place.
  • Physiotherapy may be of benefit to everyone from infancy to extreme old age.

The Importance of Physiotherapy

If pain is a problem then physiotherapy is the solution. Study and experiences prove that physiotherapy has been a very effective tool against pains and injuries ranging from minor to major. Hence the prescription of physiotherapy can never be a wrong one in case of curing pain or injury.

In case when a joint complication is to be dealt with by the help of physiotherapy, a physiotherapist may twist or fold the limbs having infirmities into positions which are not usually posed i.e. contortion may be the part of physiotherapeutic treatment.

To get rid of muscle tenancy or immobility the physiotherapist may suggest stretches, exercises, heat therapy, massage or traction and such things alone or they may be coupled and grouped as well as per the seriousness or condition of the problem. To escape from head ache or muscle ache sometimes painkiller are effective but they have limitations and side effects as well but physiotherapy deals with pains and injuries of upper tiers. Hence its importance can’t be denied in any way.

Physiotherapy not only helps us to be escaped from pains and injuries on table but it saves us from further such ailments. Healthcare professionals actually admit the importance of physiotherapy when they put stress on saying that heavy object should be picked up using knee bends instead of waist to avoid arousal of any deformity of the back bone.

The importance of physiotherapy can be observed by the fact that respiratory disease treatments are also carried out by physiotherapeutic techniques e.g. coughing, vibration, cupped hand technique, clapping etc involve the use of physiotherapeutic techniques frequently proving the importance of physiotherapy.

Whiplash is a condition due to vehicle accidents which may result in dizziness, numbness, pain in shoulder and neck muscles, ringing of bell in ear and such others. Against even these the doctors prescribe physiotherapy which may involve different sorts of techniques to rehabilitate the patient back to normal. Accident victims can be quoted while talking about the importance of physiotherapy.

Some decades earlier when the physiotherapy was growing and thriving the doctors, healthcare professionals and patients used to distrust or be indecisive regarding the use of physiotherapeutic techniques but now-a-days physiotherapy has been recognized well practiced well accepted well and physiotherapy deserves to be so.

Shortwave diathermy

Diathermy means to heat through, and electromagnetic energy can produce thermal and nonthermal physiologic effects. The new generation of swd devices are better designed and cost less. The modality is easy to use and safe.

Shortwave diathermy effectively heats large areas, while safely heating deep muscle. It matches the depth of penetration and heating rate of 1 mhz of ultrasound, with the added benefit of heating a larger area.1because swd preferentially heats low impedance tissues (e.g., skeletal muscle, blood, synovial fluid), it can heat deep muscle more efficiently. In addition, soft tissue treated with swd maintains the tissue temperature increase two to three times longer.

Ift

The interferential therapy uses mid-frequency electrical stimulus to remove the muscle sprains, strains and spasms. When applied appropriately, this mid-frequency current ways creates a massaging effect on the affected area at intermittent levels. This intermittent current stimulation causes production or secretion of endorphins and encephalins.

The endorphins and encephalins are natural analgesics which our body cells produce during painful conditions to reduce the pain. Thus by the interferential therapy, the nerves in the affected area gets stimulated leading to production of more endorphins and encephalins leading to a reduction in the pain readily. But in case of open wounds, cut injuries, infections, etc. The interferential therapy should not be used as it will increase the problems tremendously.

The interferential therapy is principally used in case of following measures:

  • To relieve the pain
  • To increase the blood flow mainly cutaneous and local blood flow
  • To stimulate the muscles so that the recuperation process will be fastened
  • To reduce the swelling and edema of the bodily parts

Ultrasound

Ultrasound is a therapeutic modality that has been used by physical therapists since the 1940s. Ultrasound is applied using a round-headed wand or probe that is put in direct contact with the patient’s skin.

Ultrasound can produce many effects other than just the potential heating effect. It has been shown to cause increases in tissue relaxation, local blood flow, and scar tissue breakdown. The effect of the increase in local blood flow can be used to help reduce local swelling and chronic inflammation, and, according to some studies, promote bone fracture healing. The intensity or power density of the ultrasound can be adjusted depending on the desired effect. A greater power density (measured in watt/cm2 is often used in cases where scar tissue breakdown is the goal.

Ultrasound can also be used to achieve phonophoresis. This is a non-invasive way of administering medications to tissues below the skin; perfect for patients who are uncomfortable with injections. With this technique, the ultrasonic energy forces the medication through the skin.

Tens

Tens is an abbreviation of transcutaneous electrical nerve stimulation.

Transcutaneous means “across the skin”. In simple terms, a tens machine stimulates your nerves via an electrical current through your skin.

A tens machine is an electronic medical device. A tens machine may assist you in modest short-term pain relief.

The use of a tens machine should be as one part of a pain management program

Pain is thought to be controlled by tens in one of two ways:

Sensory level stimulation – the gate control theory of pain means that the electrical input of the tens machine interferes with the transmission of pain signals, by blocking the neural “gate” through which the pain travels.

Motor level stimulation – the goal of motor level stimulation is to cause the release of the body’s own opiate-like substances to achieve pain relief.

Waxbath

It is learnt the benefits of thermal effects on the human body. By this method heat can be applied on the affected part smoothly and safely for perfect contact of the skin. When the affected part is immersed into the wax then that part is surrounded by the melted wax (mixture of wax and paraffin) then the part is wrapped by a polythene paper for 20 to 30 minutes for absorption of heat in the tissues.

Uses: it may be used in: rheumatoid arthritis, sudeck’s atrophy & in case of restricted movement of phalangeal joints (interphalangeal joints of hand, the hinge synovial joints between the phalanges of the fingers).
